There is nothing to close, and the rules explicitly flag blanket bans or added friction on legitimate work as a defect. Inventing new controls for failures that never occurred would only restrict the assistant without addressing any observed pattern.

The existing controls already cover the standard failure surfaces with specific mechanics — handoff notes restricted to facts and open questions, external content stamped UNTRUSTED-SOURCE and never followed as instructions, publishing gated on an approval line in `notes/approvals.md`, tool failures logged to `notes/incidents.md` with a single retry and no workarounds, and secrets never leaving the vault. Size and format constraints hold: the policy is 6 lines (well under 60), the file is under 2 KB, same shape as the original, and no secret values appear.
